Around 11,000 patients per year are likely exposed to preventable medical errors in the NHS, costing the country nearly £1 billion, according to research cited in the 2019 NHS Patient Safety Strategy In light of this information, the Secretary of State requested the health service lay a “‘golden thread’ running through healthcare.” 

The “golden thread” refers to that which goddess Ariadne advises legendary Greek myth hero Theseus to leave a trail of, so that he may find his way out of a deadly Minotaur’s cave.
